As part of supporting our colleagues, the role of the planned care team involves pathway redesign, demand management, referral management centres (RMC), and patient choice, through to commissioning and provider contract management and interface. This enables us to achieve consistency in pathways, standards, outcomes, and measures which supports system wide utilisation whilst maximising available capacity opportunities with providers.
