Keyworking team

Our designated keyworkers work with autistic people and/or people with a learning disability aged up to 25 years who are on the dynamic support database (DSD).

The dynamic support database is to identify people who are at risk or may become at risk of admission to a mental health hospital without support in the community.

To access a keyworker the person must:

  • Have a diagnosis of autism, a learning disability, or both.
  • Be 0-25 years of age.
  • Have a potential risk of inpatient admission or are currently in hospital.
  • Be registered with a GP within the geographical area of Lancashire, Blackburn with Darwen, Blackpool or Westmorland and Furness.
